   We Want Your Culture Change Success Stories

The Illinois Pioneer Coalition is committed to helping long term care communities move from an institutionalized model to an individualized model. This is often referred to as Culture Change.

Long term care communities in Illinois have made great progress towards culture change. We want to capture and share these culture change practices in a book entitled:

"Tales from the Prairie -
Culture change practices in Illinois long term care communities".

Please help us with this exciting project. Share your story of a successful culture change practice by completing a special questionnaire.

  The Illinois Pioneer Coalition was organized to provide long term care settings with the education and tools needed to effect deep culture change, thereby transforming long term care settings into real homes and improving the quality of life for individuals and staff in various settings throughout Illinois . The education provided shall be based on the philosophy, practices, and principles of person-centered and person-directed care designed to improve quality of life for elders and persons with disabilities in our society.

Our Mission

The mission of the Illinois Pioneer Coalition is to:

•  Create opportunities for communication, networking, and learning;

•  Build and support relationships and communities;

•  Identify and promote transformation in practice, services, public policy, and research;

•  Develop and provide access to resources and leadership; and

•  Assist with implementation of culture change in a variety of long term care settings across Illinois .

Our Principles and Values

The Illinois Pioneer Coalition commits to these principles and values:

•  Know each person.

•  Each person can and does make a difference.

•  Relationship is the fundamental building block of a transformed culture.

•  Respond to spirit, as well as mind and body.

•  Risk-taking is a normal part of life.

•  Put person before task.

•  All elders are entitled to self-determination wherever they live.

•  Community is the antidote to institutionalization.

•  Do unto others, as you would have them do unto you.

•  Promote the growth and development of all.

•  Shape and use the potential of the environment in all aspects: physical, organizational, and psychosocial / spiritual.

•  Practice self-examination, searching for new creativity and opportunities for doing better.

•  Recognize that culture change and transformation are not destinations, but a journey --always a work in progress.
