Illinois Culture Change Coalitions are a voluntary, grass-roots effort to drive positive changes in long term care from institutional models of care to person-centered environments. Each of the coalitions promotes the development of person-centered environments where individuals live, work and grows in nurturing, respectful communities that honor personal choice and diverse lifestyles.
